What would most psychiatrists do in this situation, regarding fees?

The doctor has a family and a lot of expenses. Running a medical office, especially if he has his own private practice is very expensive. I doubt if he is not part of a medical group that he makes any more than a skilled vocational worker. I totally understand, I do, you shared personal things and came to think of him highly and thought he really cared. He does, if he is any kind of gentleman. Let me make a comparison, when there is a disaster w/ lots of wounded ER doctors take the most likely to survive first, not the worst off. It's called triage, a life and death decision. He didn't abandon you or leave you to die, at worst he probably thought to himself he would be a little bitter and not give you the kind of care you needed if he felt he was being exploited for working for free or less than his normal wage. I don't think you should hate him, not in your heart.
